Pain Center is accepting new
patients by appointment only. Walk-in patients may only be seen if
schedule openings are available. If
possible please call our office to schedule a New Patient
consultation to guarantee that you will be seen first priority.
Patients with appointments will always be seen before walk-in patients.
Initial visits and consultations take about 20 to 30 minutes so
scheduling an appointment will help keep you from having to
wait. You can schedule an appointment by calling our office
durring office hours. |

for Pain Management
If you have recently moved to the State of Florida and do not
have a Florida ID yet we will not be able to see you until you
acquire one. Please contact your local DMV for information on
obtaining a Florida Drivers License or FL Identification Card
(ID).
- Picture I.D.
(FL Drivers License, FL State I.D... Must have Florida address
printed on it.)
- Verifiable MRI or CT Scan within the past
2
years
(If you do not have an MRI or CT Scan yet we can still help you,
please call us first and we will discuss your options before
you come in.)
- Pharmacy Report from atleast the
past
90 days
(If Applicable)
- Must Be 25 Years of Age
